Initial Assessment
60 minutes - $120
The Initial Assessment is a 60-minute one-on-one appointment inclusive of a comprehensive health history and physical exam followed by diagnosis and treatment if indicated.
The Initial Assessment is required for all new patients.
Follow Up Treatment
30 minutes - $75
60 minutes- $140
The Re-Evaluation/New Complaint appointment will include the updating of any changes in your health history, a full physical exam as well as diagnosis, and treatment, if indicated.
In order to maintain clinical and diagnostic accuracy a re-evaluation of your complaint will be conducted temporally.
